What Makes a Good Implant Candidate

If you are missing one or more teeth and wondering whether dental implants are the right solution, you are not alone. It is one of the most common questions we hear at The Loft Dental Studio, and the answer is encouraging: most adults are excellent candidates for dental implants. The key factors that determine candidacy are straightforward and well-understood.

A good implant candidate typically has sufficient bone density in the jaw to support the titanium implant post, healthy gum tissue surrounding the implant site, and good overall health that allows for normal surgical healing. Patients should be free of uncontrolled systemic diseases, particularly uncontrolled diabetes, and should be non-smokers or willing to quit before and after the procedure.

What many patients do not realize is that even those who have been told they are not candidates for implants may still qualify. A general dentist may lack the specialized training, surgical experience, or advanced imaging technology to fully evaluate complex cases. A board-certified periodontist like Dr. Ahn uses 3D CBCT (cone beam computed tomography) imaging to assess bone volume, density, and anatomy with far greater precision than standard dental X-rays. This detailed evaluation often reveals that implants are possible with the right preparatory procedures.

Conditions That Affect Candidacy

Several medical conditions and lifestyle factors can influence whether you are a good candidate for dental implants. However, very few of these conditions permanently disqualify a patient. Understanding how each factor affects implant success helps you make an informed decision.

Diabetes

Controlled diabetes is usually not a barrier to implant placement. Patients with well-managed blood sugar levels heal normally and experience implant success rates comparable to non-diabetic patients. However, uncontrolled diabetes with an A1c level above 8 significantly increases the risk of implant failure because elevated blood sugar impairs the body's ability to heal and fight infection. If your diabetes is not well controlled, we recommend working with your physician to stabilize your blood sugar before proceeding with implant surgery.

Osteoporosis

Osteoporosis alone does not typically disqualify a patient from receiving dental implants. The jawbone is denser than other bones in the body, and most patients with osteoporosis have sufficient jaw density for successful implant placement. The primary consideration is bisphosphonate medications (such as Fosamax or Boniva), which can affect bone healing. These medications require special planning and timing, but they rarely make implants impossible. Dr. Ahn will coordinate with your physician to determine the safest approach.

Smoking

Smoking is one of the most significant risk factors for implant complications. It reduces the success rate of dental implants from approximately 95% to around 85% and increases the risk of delayed healing, infection, and implant failure. Nicotine constricts blood vessels and reduces blood flow to the surgical site, which impairs the critical osseointegration process where bone fuses with the implant. Patients who smoke should quit at least two weeks before surgery and remain smoke-free during the healing period for the best possible outcome.

Blood Thinners

Patients taking blood-thinning medications such as warfarin, Eliquis, or aspirin can typically receive dental implants. Blood thinners are manageable with proper coordination between Dr. Ahn and your physician. In many cases, medications can be temporarily adjusted around the procedure. This is rarely a disqualifying factor.

Autoimmune Conditions

Autoimmune conditions such as rheumatoid arthritis, lupus, or Sjogren's syndrome are evaluated on a case-by-case basis. Many patients with autoimmune conditions successfully receive dental implants and enjoy excellent long-term results. The key factors are whether the condition is well managed and whether immunosuppressive medications might affect healing. Dr. Ahn will review your specific situation and medical history to determine the best course of action.

Age

There is no upper age limit for dental implants. Patients in their 80s and 90s successfully receive dental implants and enjoy the benefits of permanent, stable teeth. What matters is overall health, not age. The lower age limit is approximately 18, when jaw growth is typically complete. Placing implants before the jaw has finished growing can lead to complications as the surrounding bone continues to develop.

What If I've Been Told I Don't Have Enough Bone?

Insufficient bone is the most common reason patients are told they cannot receive dental implants. However, this is often a solvable problem rather than a permanent barrier. When teeth are lost or extracted, the surrounding jawbone begins to resorb and shrink over time. This natural process can leave patients without enough bone volume to anchor an implant, but modern bone regeneration techniques can rebuild what has been lost.

Several bone grafting procedures are available depending on the extent and location of the bone loss. Socket preservation grafts are placed at the time of extraction to maintain bone volume. Ridge augmentation rebuilds the width or height of the jawbone ridge. Block grafts address more significant deficiencies using bone harvested from another site. For upper back teeth, a sinus lift creates additional room for implant placement by gently elevating the sinus membrane and placing bone graft material beneath it.
